
The Day After an Explosion, Nobody Asks Technical Questions First
When an explosion occurs, the investigation almost always starts with technical facts. Equipment is examined. Installations are checked. Possible ignition sources are identified. Process conditions are reconstructed. That is logical, because explosions occur in the physical world.
But the discussion usually shifts very quickly.
The question is no longer only which item of equipment failed. The question becomes who allowed the installation to continue operating in that condition. Who knew that the plant had been modified. Who was responsible for assessing that change. And, above all, who still had the overall picture.
At that point, a technical subject becomes a management issue.
In many organisations, an installation grows slowly over time. Production lines are modified, components are replaced, control systems are upgraded and processes are optimised. Each individual decision may be rational. The process continues to run, production remains stable and the risks appear to be under control.
Until someone tries to reconstruct what the installation actually looks like today.
Explosion safety is not a snapshot. It is the result of design choices, operational decisions, maintenance history and years of small changes. When that history is no longer fully visible, a situation develops in which nobody can clearly explain why the installation was once considered safe, or why it should still be considered safe today.
That is the moment when senior management starts asking different questions.
Because by then the issue is no longer only technical. It is about responsibility, supervision and decision-making. It is about whether the organisation still has control over its own assets and its own risk profile.
In Europe, the ATEX workplace requirements under Directive 1999/92/EC place the focus on the employer’s duty to assess explosion risks, classify hazardous areas, prevent ignition sources and document the measures taken to protect workers. In the United Kingdom, the same management principle is reflected in DSEAR: dangerous substances and explosive atmospheres must be assessed, controlled and kept under review. For equipment and protective systems, the European ATEX Directive 2014/34/EU and the UK Equipment and Protective Systems Regulations define the requirements for products placed on the market, but they do not replace the operator’s responsibility for safe use, inspection, maintenance and change control.
That distinction is essential.
A certificate, a marking or a hazardous area drawing does not prove that the installation is still safe after years of modifications. Inspection and maintenance according to EN/IEC 60079-17 provide an important reality check, but they only have value when the findings are connected to the actual process conditions, the current hazardous area classification, the equipment selection, the protection concept and the way the installation is operated today.
Explosions are rarely the result of one isolated technical failure. More often, they reveal how an organisation has managed risk over time.
That is why explosion safety does not start with equipment.
It starts with leadership that wants to know how safe the real installation is today.
Not how safe it was when it was designed.
Not how safe it looked in the last document review.
But how safe it is now, after every modification, every temporary solution, every replacement and every operational compromise that has become normal.
The day after an explosion, nobody asks only technical questions.
They ask who knew, who decided, who checked, and who still had control.
